Every question is a clinical vignette — a patient presentation followed by a single best answer stem. Pure memorisation questions without clinical context are rare. Step 1 measures whether you can apply basic science knowledge to patient care, not just recall facts.

Organ systems vs. disciplines

Step 1 is organised by organ system on the day (Cardiovascular, Respiratory, Renal, etc.), not by discipline. This means a single question may test Anatomy + Physiology + Pharmacology simultaneously. Build integrated, system-based knowledge rather than discipline silos. Practice integrated USMLE questions →

How many questions is USMLE Step 1?

280 questions in 7 blocks of 40 questions each. You have 60 minutes per block and up to 45 minutes of optional break time spread across the test day.

Is USMLE Step 1 pass/fail?

Yes. As of January 26, 2022, USMLE Step 1 is reported as pass/fail only. Numeric scores are no longer provided. The passing standard is set by the USMLE program.

What subjects appear on USMLE Step 1?

Step 1 tests basic sciences in an integrated, organ-system format. Heavily tested disciplines include Pathology (~44%), Pharmacology (~23%), Physiology (~16%), Microbiology & Immunology (~9%), Biochemistry (~9%), and Anatomy/Embryology (~5%).

How long is the USMLE Step 1 test day?

7 blocks × 60 minutes = 7 hours of testing time, plus up to 45 minutes of break and a 15-minute tutorial, making the full day approximately 8.5 hours at the testing centre.

What is the USMLE Step 1 pass rate?

First-time takers from US/Canadian medical schools pass at roughly 94–96%. For international medical graduates (IMGs), first-time pass rates are typically 75–80%.

What is the question format on USMLE Step 1?

All questions are single best answer (SBA) multiple choice with four or five answer options. A clinical vignette (2–3 sentences to a short paragraph) precedes each question.

When can I take USMLE Step 1?

Eligibility typically requires completion of Year 2 basic sciences curriculum. Most US MD students sit Step 1 at the end of Year 2. The exam is available year-round at Prometric centres.

How soon do I get USMLE Step 1 results?

Results are typically released within 3–4 weeks. A pass/fail determination arrives first; for pass results, there is no further numeric information.
